What you will be doing- Executing on the population health program by engaging with patients who have clinical quality needs using standardized scripts and individual knowledge.
- Reviewing charts of patients with gaps in care to see if the gap can be closed through existing documentation, and giving providers feedback regarding documentation improvement or further documentation review as needed.
- Ensuring that records regarding clinical quality gaps from outside providers are received and documented appropriately in the EHR
- Engaging with patients through CFE messages, Next Steps, and phone calls to close their gaps in care.
- Providing longitudinal support to higher-risk patients by providing layman-level education about their codition, identifying their goals, coordinating their follow up appointments or labs, closing their gaps in care, and connecting them to resources available through their insurance company or in their community.
- Managing clinical programs (like weight management, diabetes or hypertension management, or post-ER management) by maintaining patient rosters, conducting outreach and enrollment, scheduling, monitoring eligibility, and documenting status.

What success looks like - Patients have increased utilization of primary care services directly through Eden Health or as recommended by Eden Health
- Patients close their gaps in care
- Patients have measurably improved clinical outcomes and reduced cost of care
- Providers receive feedback and education about how to close their patients’ gaps in care and consistently deliver evidence-based primary care services to patients
What you will bring- 1-2 years in a patient-facing role
- Self-starter who takes initiative to complete projects without close supervision
- High level of problem-solving, flexibility, and creative thinking
- Organized, and able to manage multiple initiatives at once while remaining positive
- Strong oral and written communication skills
- Dedication to serving patients and improving their health.
- Presume good faith among patients and providers, and communicate with empathy and compassion
- Knowledgeable in managing and manipulating spreadsheets, including pivot tables, conditional formatting, and basic Excel/Google Sheets functions
- If not knowledgeable, then an appetite and aptitude to learn
- Determined to make an impact that is measurable
- Direct clinical experience is not required, but preference may be given to candidates with experience as a medical assistant, community health worker, health coach, or a similar role

$49,600 - $62,000 a year
The base salary range for this role is $49,600 to $62,000 and is determined based on experience, geography, and qualifications. The base salary is one component of the total compensation package, which includes equity, PTO, and other benefits. This position is salaried, non-exempt, with overtime pay eligibility.

While the majority of Eden Health positions are remote-first, we are a company that values building trusted relationships.
