About the Opportunity

Pair Team is building a team of deeply passionate individuals ready to change primary care operations for those who need it most.  We are looking for a highly motivated full-time Bilingual (Spanish) Onboarding Specialist who is willing to think creatively and empathically to help our team change the way people access healthcare. They will play a critical role on our team by directly engaging with and onboarding individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide. 


What You’ll Do:

  • Support the care team by intaking new patients into the Enhanced Care Management (ECM) program by gathering key social, behavioral, and clinical information
  • Provide clear communication with the patient and make sure they fully understand the ECM program features and benefits
  • Create a safe, compassionate environment to build immediate rapport and trust with our patients 
  • Coordinate a clean hand-off to a Lead Care Manager and schedule their new patient introduction meetings
  • Assist with operations and behavioral health quality improvement projects to optimize the process for our patients and team


What You’ll Need: 

  • 1+ years of experience working in a fast-paced healthcare environment providing administrative support and intaking new patients - experience with gathering key patient information, documentation, and scheduling appointments  
  • You are results driven with a proven track record of exceeding performance indicators
  • Bilingual with excellent communication skills with proficiency reading, writing, speaking, and listening in both English and Spanish
  • You have strong technical skills and comfort with utilizing multiple technology platforms
  • Requires a quiet, HIPAA compliant and internet connected space
  • Dedicated to supporting and improving the care for many underserved populations
  • You are a fantastic listener, skilled at “reading people” and understand how others may be feeling or thinking based on nuances, uncomfortable silences, or questions they ask
  • You are reliable and comfortable in an environment that is ever-changing to improve the wellbeing of underserved communities through increased access to high-quality care
